Creating an Aphasia ID Card

From The Aphasia Center:

(can be printed in: English, Spanish or Norwegian)

  • Not laminated or in plastic cover (can do this yourself)
  • You Can customize and then print from your computer
  • Says “I have Aphasia” and explains what aphasia is
  • To fill out:

-You type in your personal information.

Customize by picking from a predetermined list of techniques that can be used when communicating with you

Customize by typing in the cause of your aphasia (ie: stroke, PPA, Brain Injury).
